A standard sheath introducer system comprises several integrated components engineered for functionality and safety:

  • Dilator: A tapered inner obturator that smoothly expands the tract following needle puncture, facilitating the atraumatic advancement of the sheath into the vessel.

  • Sheath: A thin-walled, kink-resistant tube (available in various French sizes and lengths) that resides within the vessel, providing a secure access port. It is typically made of biocompatible polymers like PTFE or polyurethane for minimal friction and thrombogenicity.

  • Hemostatic Valve: A one-way valve (e.g., diaphragm-type or Tuohy-Borst adaptor) integrated at the proximal end. It prevents back-bleeding and air embolism while allowing the free passage of devices and facilitating fluid flushing.

  • Side Port: An integrated luer-lock port connected to the sheath lumen, enabling continuous saline flushing, administration of medications or contrast media, and direct pressure monitoring.

The sheath introducer’s core utility lies in addressing fundamental needs of endovascular procedures:

  • Procedural Efficiency & Safety: By providing a dedicated, sealed conduit, it streamlines device exchanges, reduces procedure time, and allows for continuous infusion to prevent thrombus formation within the sheath. The side port facilitates easy aspiration to remove air or clots.

3. Indications, Sizing & Considerations

Sheath introducers are versatile tools used across a wide spectrum of procedures with specific selection criteria:

  • Common Clinical Indications:

    • Diagnostic and therapeutic cardiac catheterization (coronary angiography, PCI).

    • Peripheral vascular angiography and interventions.

    • Electrophysiology studies and ablations.

    • Central venous access for catheter placement.

    • Neurointerventional procedures.

  • Sizing & Selection: Sheath size (French, or Fr) is chosen based on the outer diameter of the largest device to be introduced. Common sizes range from 4Fr to 24Fr. Length is selected based on access site (e.g., shorter for radial artery, longer for femoral access).

  • Material & Coating: Modern sheaths often feature hydrophilic coatings to reduce friction and enhance trackability, especially for radial or tortuous anatomy. Antimicrobial coatings are also available to reduce infection risk.
